This “Report Accessibility Barriers” form is for reporting Accessibility issues related to the Americans with Disabilities Act (ADA). ADA barriers can be physical barriers such as missing curb cuts, ramps, or inoperable elevators - or they can be electronic - systems that do not work properly using screen readers or other assistive technologies. This form is NOT for not for reporting problems registering or fpr requesting assistance with other  routine administrative or technical problems. For technical issues, please contact Campus Technology Services by creating a Work order. To request support for routine administrative matters, please contact the Enrollment Services office. 

Please use this form to report any ADA access barriers you encounter, either physical or electronic, such as an inoperative elevator or wheelchair lift, bicycles blocking an access ramp, drifting snow, or inaccessible documents on websites or electronic information in classrooms that is not accessible, so the College can remove those barriers as quickly as possible.

Reports about barriers should be made using the form below. The information you submit through this form goes to the Accessibility Team. We will attempt to follow up in as timely a manner as the information provided permits.

If you are not satisfied with the response to submitting this barrier report or if you want to invoke another procedure, you may file a complaint under the College’s Nondiscrimination Policy.
