Required Health Forms for Incoming Students
Don’t forget to complete your required forms prior to starting the Fall semester.
Please complete your health history and vaccination forms prior to starting the Fall semester.
These forms include:
- Patient Health History Form
- MMR vaccination form
- Physical exam and immunization form, to be signed by medical provider
- Minor Consent Form if student is less that 18 years old
You can submit these forms by:
- Completing the Health History and MMR forms on the patient portal
- Upload the signed Physical Exam and Minor consent form to the patient portal.
