Required Forms
For all summer youth and precollege programs:
- Youth Registration Form and/or Precollege Registration Form
- Credit Card Authorization
- Legal Disclaimer
- Code of Conduct
- Health Form: This form must be completed and signed by a parent or guardian and accompanied by a (camp/school) health form with immunization history signed by your family physician before your child may begin his or her program. *not required for remote learning.
Additional Forms
To be completed when appropriate:
- Self-Administer Release
- Driver Permission - Permits must be obtained in person (not available online) at the Parking and Transportation Office in Campus Center North during their business hours.
