International Transfer Form - This form is required only from students currently attending high school, university or a language institute in the U.S.. The completed form should be sent, along with a photocopy of your visa and I-20 form, if and when you are accepted to Purchase.

Supplemental Admissions Forms
SUNY Supplemental Application and Essay: Freshman Liberal Arts and Science applicants and Freshman Art+Design applicants using the SUNY application must complete the SUNY Supplemental Application and submit the essay answer for one of the essay topics (your choice) in the SUNY Supplemental Application. Music, Dance and Theatre Arts applicants are not required to submit the SUNY Supplemental Application.
The Common Application Writing Section: Freshman Liberal Arts & Science and Art+Design applicants using The Common Application must complete the Writing requirements Short Answer and Personal Essay sections in The Common Application, instead of the SUNY Supplemental Application and Essay.
Transfer Supplemental Form: Transfer applicants for Liberal Arts and Sciences, Liberal Studies, and Art+Design must complete the Purchase College Transfer Supplemental Form. The form is available as a PDF: Transfer Supplemental Form 09-10 or as a Microsoft Word document: Transfer Supplemental Form 09-10. Be sure to complete all applicable sections of the form including the required statement. The supplemental form should be emailed directly to admessay@purchase.edu.
